The reliability and validity test of the Chinese version of the Tyndall-Bailey Falls Risk Assessment Tool for ICU

Objective To sinicize Tyndall-Bailey Falls Risk Assessment Tool For ICU (ICU-FRAT) and test its reliability and validity. Methods Based on Brislin model,the original scale was translated,back-translated,checked and translated,cultural adjusted and pretest to form the Chinese version of ICU-FRAT. A total of 200 ICU patients from 3 hospitals in Nanjing from March to May 2022 were investigated by convenience sampling to evaluate the reliability and validity of the scale. Results The Chinese version of ICU-FRAT has a total of 10 items,with item level content validity index of 0.83 to 1.00,overall consistency level content validity index of 0.90,and average level content validity index of 0.98. After exploratory factor analysis,3 common factors were extracted,and the cumulative variance contribution rate was 79.32%. Confirmatory factor analysis suggested that each test quantity of scale model fit was within an acceptable range. The Cronbach’s α coefficient of the total scale was 0.857;the Cronbach’s α coefficient of each item ranged from 0.830 to 0.867;the half reliability was 0.838. Conclusion The Chinese version of the ICU-FRAT has good reliability and validity,and it can be used to assess the risk of falls in ICU patients.

The effects of health literacy on decision satisfaction of decision-makers in patients undergoing cardiac macrovascular surgery

Objective To explore the mediating effects of perceived stress and uncertainty tolerance on health literacy and decision satisfaction of decision makers in patient undergoing cardiac macrovascular surgery,and to provide references for improving the satisfaction of decision-making. Methods 260 surgical patient’s decision-makers in the cardiac macrovascular surgery of a hospital in Wuhan were selected by convenience sampling. All Aspects of Health Literacy Scale,Perceived Stress Scale,Intolerance of Uncertainty Scale-12 and decision satisfaction scale were used to investigate. Results 251 valid questionnaires were recovered,with an effective recovery rate of 96.54%. The health literacy score was(23.05±5.61);the perceived stress score was(20.31±6.08);the uncertainty tolerance score was(26.41±5.94);the decision satisfaction score was(43.86±10.54). The 4 variables were correlated(P<0.001). The mediating model found that health literacy had a significant direct effect on decision satisfaction. Meanwhile,health literacy also indirectly affected decision satisfaction through 3 paths,including the independent mediating effect of perceived stress(β=0.096,P<0.001),the independent mediating effect of uncertainty tolerance (β=0.093,P=0.001),the chain mediating effect of perceived stress and uncertainty tolerance(β=0.076,P<0.001),accounting for 49.17% of the total effect. Conclusion Perceived stress and uncertainty tolerance play a mediating role in the relationship between health literacy and decision satisfaction of decision makers in cardiac macro-vascular surgery,which can improve decision makers’ health literacy,reduce perceived stress,and help them perceive surgical risks rationally,thus improving decision satisfaction.

Translation and test of validation and reliability of Inflammatory Bowel Diseases Disk Scale

Objective To sinicize the Inflammatory Bowel Diseases Disk (IBD-disk) Scale into Chinese,and to test the reliability and validity of the scale. Methods Through literal translation,back translation,expert discussion and evaluation and cross-cultural debugging of the scale in English,2 researchers finally formed the IBD-disk Scale in Chinese. 315 patients with inflammatory bowel disease who were followed up in the outpatient department of a tertiary A general hospital in Anhui province from June 2021 to August 2021 were selected as the research subjects. 4 weeks later,48 patients were selected by a random number table method for retesting. Results The Chinese scale contains 10 items,and the correlation coefficients between the scores of each item and the total score of the scale were 0.630~0.816(P<0.001). The CR values of 10 items were 4.79~30.93 with statistically significant differences(P<0.001);exploratory and confirmatory factor analysis supported the one-dimensional nature of the scale,and the fit was good. The internal consistency coefficient was 0.895,and the retest reliability was 0.861,which indicated that the data reliability and quality was good. There was a positive correlation between IBD-Disk Scale and IBD Burden of Disease Scale(r=-0.513,P<0.001). Conclusion The Chinese version of IBD-Disk Scale has good reliability and validity,and it can be used as a tool to evaluate the degree of disability in patients with inflammatory bowel disease.

Deep vein thrombosis of lower extremity in ICU patients in plateau area:a cross-sectional study

Objective To investigate the status of deep venous thrombosis(DVT) of lower limbs in ICU patients in plateau area,and to analyze its influencing factors. Methods The convenience sampling method was adopted to select ICU patients in a tertiary A hospital in Qinghai Province from May to October 2020 as research subjects. The general demographic data and clinical data of the patients were collected. Doppler color ultrasound was used to diagnose the venous thrombosis of both lower limbs in ICU patients. The independent risk factors of DVT in ICU patients were analyzed. Results A total of 192 patients were investigated. The incidence of DVT in ICU patients was 10.42%. Univariate analysis showed that gender,ethnicity,age,altitude of residence,diagnosis,Acute Physiology and Chronic Health Evaluation Ⅱ,APTT,urea nitrogen and C-reactive protein were associated with lower extremity DVT,and the differences were statistically significant(all P<0.05). The results of multivariate analysis showed that female(OR=9.094,95%CI 2.035~40.639,P=0.004),ethnic minorities(OR=5.510,95%CI 1.082~28.052,P=0.040),the altitude of residence 2 000~2 500 m(OR=4.395,95%CI 1.433~44.572,P=0.021),APTT>37.05 s(OR=6.727,95%CI 1.117~40.496,P=0.037),C-reactive protein >3.07 mg/L(OR=9.545,95%CI 1.403~64.945,P=0.021) were independent risk factors for DVT in ICU patients. Conclusion Early screening of DVT should be carried out for patients admitted to ICU,and the ICU patients who are female,ethnic minority with APTT>37.05 s and C-reactive protein>3.07 mg/L at altitude of 2 000~2 500 m should be regarded as the focus group of concern for DVT.

Effect of Return-to-Family intervention on rehabilitation efficacy of breast cancer patients

Objective To explore the effect of “Return-to-Family” intervention program on rehabilitation effects of breast cancer survivors. Methods A total of 70 breast cancer survivors were recruited by convenient sampling in Breast Surgery Department and Breast Nursing Clinic of a tertiary hospital in Beijing. They were divided into a control group and an intervention group,and each group has 35 patients. In the intervention group,the program of “Return-to-Family” intervention was carried out. In the control group,the routine care was provided. The effectiveness of the intervention program was assessed before and after intervention by the function of shoulder joint and arm at affected side,the Disabilities of the Arm,Shoulder and Hand questionnaire(Quick-DASH),the Cancer Rehabilitation Evaluation System-Short Form(CARES-SF),the Family Adaptability and Cohesion Evaluation Scales (FACES). Results After intervention,the total scores of the function of shoulder joint and arm at affected side,the FACES in the intervention group were higher than those in the control group and those before intervention(P< 0.05). The total scores of the Quick-DASH and the CARES-SF in the intervention group were lower than those in the control group and those before intervention(P<0.05). Conclusion “Return-to-Family” intervention benefits the improvement of upper limb function,quality of life,family function for breast cancer patients,and help breast cancer survivors return to family smoothly.

Evidence-based practice in prevention of fecal incontinence associated dermatitis in ICU patients

Objective To summarize the evidence for the prevention of fecal incontinence associated dermatitis in ICU patients and to evaluate its application effect. Methods To obtain the best evidence,we used the method of evidence-based nursing to search related guidelines,expert consensuses and systematic reviews from domestic and foreign databases from January 2010 to September 2019. The best evidence was evaluated and summarized;review indicators were developed;the baseline review was conducted,and facilitation and obstacles based on the review results were analyzed. From May to September in 2020,the evidence was applied to clinical practice,and the incidence of incontinence associated dermatitis,nurses’ cognitive level of incontinence associated dermatitis knowledge and the implementation rate of review items were compared before and after the application of evidence-based practice. Results Finally,8 articles were included,including 3 clinical practice guidelines,2 expert consensuses and 3 systematic reviews. 11 pieces of best evidence were summarized in terms of risk assessment and prevention strategies. After evidence-based practice,the incidence of incontinence associated dermatitis was 19.10%,which was lower than that before evidence-based practice (40.48%) with statistically significant differences(P<0.05);the score of nurses on incontinence associated dermatitis knowledge questionnaire was(42.35±5.77),which was higher than that before evidence-based practice (35.65±5.07),and the difference was statistically significant (P<0.001). The implementation rates of items 2,4,5,6 increased from 0,0,66.67% and 22.62% to 100%,84.27%,97.75% and 76.40%,and the differences were statistically significant(P<0.001). Conclusion The application of evidence-based practice in the prevention of fecal incontinence related dermatitis in ICU patients can reduce the incidence of incontinence associated dermatitis,improve nurses’ cognitive level and the implementation compliance.

A survey of the position management status for orthopedic specialist nurses

Objective To investigate the current situation of position management of orthopedic specialist nurses(OSN) in China,and to provide references for the formulation of relevant management systems. Methods A self-designed questionnaire was used to facilitate the investigation of the status and satisfaction of the post management of 560 OSN in 28 provinces/municipalities across the country. Results In terms of the management system of OSN,there were only 49.29% of the participants whose hospitals had special management systems for specialist nurses,and 20.89% of the hospital management system was not transparent. Furthermore,the specific rules in employment,level management,performance appraisal,continuing education and training,and post incentive system in those hospitals were different. As for the postpractice of OSN,74.64% of the participants didn’t change their work positions,and their main jobs were still direct clinical nursing practices,but the development of orthopedic advanced nursing practical activities were limited. Job management satisfaction was 56.96%,and only 9.46% had the intention to leave. Most of them(72.86%)were confident about the development and prospects of OSN. Conclusion The satisfaction of post management is at a medium level,as it is at the initial stage,and the advanced nursing practice activities are gradually carried out. The whole team has high professional commitment and considerable development and prospects. It is necessary for the state,hospitals and individuals to work together with their efforts to establish a scientific,rational and perfect system for postmanagement of OSN,so as to improve the growth and strength of OSN team.
